1st CLASS CARE will use its best efforts to refer you to one of our team of face to face doctors and other medical practitioners experienced in chronic pain management and other medical problems. Our clients have reported that our references have helped them to cope with sciatic nerve, back, shoulder, neck , muscle and abdominal pain. The practitioner to whom we refer you will perform the face to face consultations and the appropriate follow up telephone consults as is proper under local law, good practice and the Ryan Haight law. Our doctors and other medical practitioners are knowledgeable about Schedule 3 medications (e.g. hydrocodones), but prescribing them as well as setting the number, type and timing of face to face or follow up consultations is completely up to the practitioner's professional decision, taking into account said laws and practice mandates.
We are NOT a pharmacy and do not sell, offer, dispense or ship any medications, including pain medications. You must take any prescription resulting from a consultation to the pharmacy of your choice - upon request we will try to help you in this. Our service requires that you submit to us good medical records, a legible I D and our Introductory Medical Information / HIPPA release form filled out and manually signed. In accordance with the "Ryan Haight Online
Pharmacy Consumer Protection Act of 2008" our consultation cycle can begin only with an in person face to face medical evaluation by the practitioner who issues a prescription to you for a controlled substance. The practioner will decide upon the number and timing of all subsequent "follow up" telephone consultations as may be required by good practice and local law. The initial in person medical evaluation will take place in the state in which the practitioner is licensed. We encourage the practitioners who work with us, and subject to their professional decision, to follow a "no refill-one month follow up" protocol, to include necessary blood work in their exam and to secure a patient agreement when appropriate."
1st Class Care complies with the requirements of Section 311(c) of 21 USC 802 with respect to the delivery, sale or offer for sale of controlled substances. To become a client, click on the NEW CLIENTS box above and submit to us a completed Introductory Medical Information form (IMI) together with your medical records and a legible ID. Upon receipt of these items you will be contacted by one of our Customer Service Associates by email, fax or phone. They will discuss with you the material you have submitted and our consultation and other procedures. If you have not been contacted within 24 hours of your record submission, please contact us. Upon approval of your IMI, ID and records, you will be a client. If you want pricing explanation, call us. Medical records must support your complaint and have a diagnosis from a medical practitioner which diagnosis can not be more than one year old; CT scans, MRI's or X Rays must have practitioner's comments on them. Your records must establish that your condition is chronic and continuing. As stated, your medical records must show a practitioner exam within one year of beginning our service. The records must also state the medication ordered/suggested by the practitioner or that it is known to him/her that you are taking same. We can NOT arrange a consultation for you unless you have given us adequate records. Also, WE REPEAT - in order to begin your consultation cycle with us you must have a NEW in person face to face medical evaluation by our referred practitioner. Also, you should be able to describe your symptoms – some of which we have heard from other clients such as “leg pain, back pain, tingling, numbness, muscle weakness and travelling or stationary.
- You must give us a LEGIBLE photo I. D. including a physical description and a new address (example – Driver’s License). Make sure you keep the I. D. / Address up-to-date.
- As part of our continuing effort to prevent diversion of controlled substances, we go the extra steps to confirm the authenticity of the medical records submitted to us and accordingly may contact the office of the health care provider mentioned in those records.
- If a prescription results from your initial face to face in person consultation, the practitioner will deliver it to you. Any prescription resulting from a follow up consultation will be sent to you. In all cases it is up to you to take the prescription to the pharmacy of your choice. In some cases, at your request, scripts may be sent to your pharmacy. 1st Class Care does NOT dispense, sell, offer to sell or ship any medications; it is NOT a pharmacy in the usual sense - we arrange to help you.
- In order to reduce the risk of diversion, we strongly suggest to every practitioner that the scripts are prepared on tamper proof and copy revealing forms.

Finally, a few preliminary notes:
- Payment to us is by credit card or COD (Money Order or Cashier’s Check). We ship documents UPS. For your added convenience we have arranged that you can send your payment to us (even using a credit card) via Western Union.
- The medical practitioners we work with are all located and licensed in the U.S. and prescribe/supply only FDA approved U.S. source products.
- You must read and agree to our “Terms and Conditions” and “Privacy Policy”. If you agree, click on the “AGREE” box at the end of the “IMI Form.”
- When the time comes for a re-consult, we will ask for information about any interim changes in your condition.
- Our Doctors are experienced in this face to face process and are used to dealing with pain problems.
